The link is broken between our form and our spreadsheet

need help reconnecting them.  it looks like everything is set up

I have pushed the submissions to the spreadsheet for the form embedded on your web page. Please check if the new submissions are being updated in the spreadsheet now. Please note that any modifications made to the integrated spreadsheet will break the integration and the submissions cannot be updated. In such cases, it is required to remove the Spreadsheet integration on the form and reintegrate again so that the submissions will be updated normally.
