I filled out a form for www.sleephealthmd.com. I now see that it was a mistake to do so. Your company sent all my VITAL and PRIVATE information to an unsecured Google email account? I'm keeping this feedback, the email, and all materials available and sending it to my lawyer just in case. I cannot believe this company would do this in this day and age. Re-review your processes, policies and privacy statements before you expose more innocent people to fraud in the way you have.

I checked the link you have provided and I noticed the form on the site which is (https://form.jotform.com/62875100551148) is owned by one of our JotForm Users. Can you please confirm that you have filled out this form (https://form.jotform.com/62875100551148)? If yes, then the data is sent to the Form Owner and we do not share this data with other parties because submissions to a form is a responsibility of the owner of that form.
