Duplicate a set of field (or add item button with fields)

Hi there,

This may have been asked but I haven't found anything concrete. I'm looking to have the option to add an item button, where the user can click ADD Item and a new set of form fields are added, like Name, Address, phone etc

As like in this example below

Is this possible with Jotform? or through a thrid party widget?

You can use infinite lists widget with calculations, it can give you row number and you can use it to calculate total ticket price for instance.
