If you have a pre-designed form (PDF) for example. Can you upload that document to jotform to create the template or do you have to re-create the form in jotform manually?

You may upload a PDF form and convert it into an online JotForm form by using our Fillable PDF Creator.
Just head over to this page https://www.jotform.com/fillable-pdf-form-creator/ > click the orange CREATE A FILLABLE PDF FORM button > then pick the middle option Convert PDF Form (Have an existing PDF Form? Make it fillable and submittable).

I also strongly urge you to read the guide above so you're aware of the limits of PDF forms. If the PDF form is a bit complicated and can't be converted properly, then I'm afraid you'd have to manually build the form from scratch.
