Have you modified the Google Spreadsheet? The integration might not be friendly to the spreadsheet customizations (for example, if you moved some columns, renamed or deleted them).
If you redo the integration, it should create a brand new spreadsheet with complete form data.

spreadsheet after renaming them and it worked fine

Unfortunately, any modifications made to the integrated spreadsheet will break the integration. Please remove the existing spreadsheet integration on the form and re-integrate again and it should be working fine. If you need to work on the data captured to the spreadsheet, it is recommended to copy the data to a different spreadsheet.
