Why form submission fields are not matching the form?

I've set up a form: https://form.jotformeu.com/70592414097358

However when someone fills out the form and we receive it back, the questions are totally different? 

I checked your Email settings and I found out that the labels are not matching:

So you can either manually change those labels or you can simply delete the notifier and create another one, it will pick up the fields automatically.
