Why has my account been suspended?

When I go to login into my account to view my form, I received a message that the login in attempt has failed and that the account is suspended. About two weeks ago I had to answer some Jotform account questions, regarding the use of the form and what website the form is attached too, but once completed I was allowed access to my forms. But now I need to once again access my forms to make some changes and my account is now suspended and I was never notified of it or why.
