I am not able to upload an image into a form! :)

I am currently building a form, and I am trying to insert an image and it doesn't seem to be working. I have tried inserting several different types of images (jpeg, png, etc), and all I get is the little picture with a mountain/sun to show it didn't work. 

I tried originally to insert this image into a header with text, and it didn't work. I tried then to drag over the "image" box from the widgets, and wasn't able to get that to work either. :)

I've tried to upload an image on a cloned form of yours and I can see it works: 

I would suggest you to rename your file, please remove  the spaces and all special characters from it, that may be breaking the field's link. 

If you cannot upload the image, please provide us with that, you could directly upload it on this forum post by following this guide or you could upload it to a third party cloud service and share the link with us.
