It's possible by downloading your form's full source code, then edit the HTML file and insert this code between <head> </head> tags.
<link rel='shortcut icon' target="_blank" href='http://www.nannynest.com/favicon.jpg' rel="nofollow noopener" /><link rel='icon' target="_blank" href='http://www.nannynest.com/favicon.jpg' rel="nofollow noopener" />
Let me know if you need further assistance. Thank you.

I've already added your website favicon on that code :). What you need to do is to insert that code on the HTML file, insert it just below <head> tag. You can find this tag on the top, probably on the 2 code lines.
Let me know if you need help. Thanks.

The word does not appear in this source code, I followed every step.
I went to embed, source code, I copied it into another document to edit
and I have done a search for the word "head" and it only appears once (look
below I have highlighted it). I see that the second link in the email was
the favicon-thanks. Please show me where that link should be inserted in

I think there's a problem with the guide, you can download your form files by clicking on "Click Here" button while you are on the source code window. Check this: http://pasteshack.net/images/067137001341370309.png
Anyway, I have edit it for you. You may copy the full source code here with your favicon, save it as HTML file and upload it to your website. You can check the favicon code there.
Hope this helps. Thank you.
