    My form is showing up incomplete

    I don't understand why this is happeneing at all. But my form is showing up incomplete. When I edit the form it looks fine but here is the code: <script type="text/javascript" src="//form.jotform.us/jsform/21688365230152"></script>. It looks incomplete on my site, why?

    Hi

    You would need to modify a little bit the URL of your form (as shown in bold), also try replacing the current javascript code with the iFrame code :

    <iframe allowtransparency="true" src="http://www.jotform.us/form/21688365230152" frameborder="0" style="width:100%; height:827px; border:none;" scrolling="no">
    </iframe>


    You can copy and paste the one above, if you form still does not show up correctly in your site , let us know

    Hi

    In order to get the iFrame code , please follow the example on this guide , for any form you would like to modify.

    However if you stll want to modify the form involved in the original post, simply copy and paste into your page, the code in green above

    Of course you would need to delete the current code:
