Form gone, revisions and submissions gone?

Something happened to this form https://www.jotform.com/build/70856573583973

I wanted to look back at the previous form dollar amounts.  Instead the form and everything related to it vanished and the form renamed itself.  I need it and the submissions back, but the revision function fails.

We urgently need your help.
