I've been working on the form all evening, and a few hours ago the page refreshed to Form Not Found This form is disabled. The settings state the form is active- is there something I missed?

We would like to apologize for any inconvenience this had caused you. I have checked the status of your account and it is on auto-suspension status. It is possible that one of our forms was suspected of doing phishing activity that is against our terms (Terms-of-Use). I, however, reviewed your form and verified that everything seems good.
You should be able to access your forms again as I have reactivated your account.
