Missing submission payment in form

I sent someone the form and I received the pay pal transaction but not the form signed. I asked if she filled it out and she said yes so I am wondering where I can recover the form. My clients name is Keeley Kleist.

Is there an equivalent email of the missing submission payment?  Successful payments should also have email notice.
