How can I make my form look like this one?

to look like this:

I've tried copying some of the css from other help responses, but I'm still getting some of the fields aligned left.  I also don't want the subheadings to be that orange-ish color, and would like to have the boxes be taller like they are here.

Additionally, is there any way to make my subheading italicized?

Can you share with us the link to that sample form? This way, we can copy its styles especially the colors and spacing. I would also suggest adding what you want to add in the form such as the heading. This way, we can include the styles to that fields.

There is a service specifically for building mobile forms (APPS) that works on all mobile devices - you design a mobile form that is linked to your existing paper form. When you submit the mobile form - it will automatically fill out your linked paper form and email a completed PDF to you.

You can use the mobile device to take photos and they will be embedded in the PDF. You can set the email subject line to auto fill specific fields from your form for easy identification. You can send copies of the PDF to a specific Google Drive folder, Google Sheet or Box folder.

Its an APP - so no headaches trying to use Adobe or Excel or other software on a mobile phone!
