Please allow me to inform you that adding a Password Field on your form to ask passwords from the users is against our Terms of Use. If you use a password field and it violates our Terms of Use, your account will be suspended resulting to forms being disabled.
If you want to limit the users to access your form, you may use other labels such as "Access Code" or "Access Key".

You can integrate your form with PayPal to accept payments and embed your form to your website.
