Do you have a form that will do the following...

I want customers to send me an image file. I will print that image on a product such as a pillow cover or canvas print. They send the image, pay for the product, then I create the product and send it back to them. On my website I can accept credit cards and paypal. Thanks so much if you can recommend a form that will do this and take payments.
