The form appears to keep the results cached and it never refreshes.

Also, in testing the form appears to keep the results cached and it never refreshes.  For example, if I choose qty 1 of item number ABC and qty 3 of item number DEF, those results never go away.  The next person that touches the form still has those qtys selected instead of being able to start from zero.

I was not able to replicate mentioned issue. Each time when I reloaded the form quantity options were reset back to zero. 

Previously entered data would only be saved if Auto Fill feature on the form is enabled. 

How to Enable Auto Fill on Forms

I have checked and this feature is disabled on your form. If it was previously enabled, because of that you were seeing previously entered data. 

However even if this feature is enabled your users will not see data that you have entered when they open the form, because data is saved locally, in browser cache. 

If you still have issues on your end and you see previously entered data, try to use some other browser or clear your browser cache.
