Why is the table in the form not properly formatted when viewed on mobile?

    The IMPUT TABLE are showed on smartphones as following. Can you help me please in solving the issue?

    I tested your form on my phone (Android) but I wasn't able to replicate this problem. Anyhow, I inserted a Mobile responsive widget in your form, please check now and let us know if that helped. 

    If it doesn't work for you, please let us know the type of phone you used to view the form so we can check further.
