You acknowledge having received my form, but never were there a account created. Why not?

It has been over a month ago since I fill the form out.

The form you filled out was created by one of our users.  If you believe the form was legitimate, contact the person who created the form or the site owner of where you found the form.  On a side note, would you mind letting us know what type of form it was and where you filled it our from?  I suspect it may be a phishing form but would just like to confirm before suspending the account.
