    Prepopulating form fields through URL in email notification

    What you sent me discusses a client filling out one form and then going to the next screen and the data carries over to the send form and prepopulates the data you map to it.
    I need to collect data from an intake form that a patient completes. I need that same information to be prepopulated into a totally separate interaction on a form that is for the doctor. When the doctor fills out his text box with information then they submit the form and automation goes on from there.
    I am trying to avoid having to have the doctor fill out all the information on a new form when we already have it. He just needs to fill in his opinion in the box.
    Does that make sense?

    If that is the case, then I suggest you pass the "prepopulated link/url" in the email notification and make the "Doctor" as the email recipient. 

    In the Email Content, you should insert the prepopulted link/url in the body. Please check these guides:

    Ok first I want to make sure I am understanding this correctly.
    I want to take form “Client Intake” attached and whenever a client goes to my site and fills that information out, I want it to take like 8 or 9 fields only and prepopulate the fields on the other form attached called “Interpretation”.
    Once interpretation form is prepopulated, are you saying I can have that emailed to the doctor abd when they click the link the prepopulated form opens up and he/she types in the one big text box only and hits submit. Then that will submit the completed form back to jot form?
    Please confirm.

    Yes, that is correct. The clients will fill out the "Client Intake" Form and once submitted, the data on that form will pre-populate the fields on the other form which is "Interpretation". 

    You can also create an Email Notification on the form "Client Intake" where the doctor is one of the recipients so that he/she will receive an email alert with the prepopulated form link.

    Then, he/she will only need to click the link and the prepopulated form opens up and he/she types in the text box on the second Form "Interpretation". And lastly, that will submit the completed form back to Jot form.

    Email notifications I get but how to do what you confirmed below and understand what I am trying to do I do not understand how to do it.
    Your description below is exactly what I want to do. What do I need to do to the Intake form to get it to prepopulate the exact data needed into the interpretation form so the doctor can simply fill out the text box?

    From the previous post of my colleague, he already gives you the steps to take on how to accomplish your requirements. 

    1. It explains in this guide on Setting Up Email Notifications. Add the doctor's email as one of the email recipients.

    2. In the Email Content, please insert the prepopulated link/url in the email body. 

    This guides will help you to create a prepopulated link/url:

    Prepopulating Fields to Your JotForm via URL Parameters

     How to Automatically Pass Form Data to Another Form

    For example: 

     

    So, when the form is submitted by the clients, the doctor will receive a prepopulated link. When the doctor clicks the link, the Interpretation form will open and the doctor will fill out the form.

    3. Then, once the doctor filled out the form, that will submit the completed form back to Jot form.
