Form not found - This form is disabled.

I read after mt forms were disabled that Im not supposed to collect payment information without using the widgets but my forms are still disabled after I removed the elements.

This could be due to the form http://www.jotform.us/form/71006156490147 which seems to be collecting sensitive information before. But I just checked and the guilty fields are no longer there so I reactivated your account.
