my survey forms are disabled for some reason?

when I click view form, it is takes me to a page that says form disabled, but my forms are not greyed out which means disabled?

As already explained by my colleague in the other thread your account got autosuspended because your form "RebelFish Local Social Media Cover Logins" is collecting usernames and passwords, in violation of our terms of use.
