Cannot delete form field

I have tried on multiple browsers to delete 2 fields that I do not want on my form but it is not working. The fields are the last 2 on this form.

In Form Builder V4, you can delete a field by clicking the "Remove/Delete" button while the field is selected. Here's a screen capture:
