I've taken a look at the problem you aren't able to find and found that it is on a different account under the username "JSor".
According to our system you have 11 different accounts so you were probably logged into that particular account at the time that form was created.
We can move it to this account for "kbarlow84" (per this thread) at your request if you wish.
