I've set up an account for a client and created a survey form: http://form.jotformeu.com/form/21717009273349
I need to change settings and I can't find login details for this account. I think the username was cmcoaching. But when I used password reset neither myself nor my client received the password reset email.
My addresses could have been info@, tamara@ tjconsulting.co.uk or email@example.com.
Client's address is firstname.lastname@example.org.
Could you please help us restore access for the form!

That's correct! That account belongs to cmcoaching account but the e-mail address is email@example.com, so I guess this is wrong. I then went ahead and changed it to firstname.lastname@example.org, you may now recover your password by following this guide.
