Is there a way to create a login for each employee?

Is there a way to have a login for each employee or some sort of passcode in order to ensure that the employee is submitting their own reports?

Unfortunately it is currently not possible to create a login form in JotForm but with a little workaround it is possible to achieve your requirement. You can take advantage of our "Conditions" feature to show / hide form fields based on the input of any text box.
