How to add logo in form?

I click on add logo and nothing happens

There are two options to add logo image in form:

#1. Add an "Image" field in form to add the logo URL. Please check the screenshot below:

#2. You can also add "Header" field in your form and set the image URL. Please check the screenshot below:
