URL of page form is embedded on

I would like to create a feedback form for all our knowledgebase articles (hundreds of them). Is there a way to record the link (or title) of the page the form is on and provide that as part of the submission?

Yes, it is possible to get the page information where the form is embedded. Please use the Get Form Page URL widget. This widget detects the webpage where you embedded the form and provides the page URL each time the form is submitted.
