Please wait... not able to submit form

    I have tried submitting to

    multiple time and only receive "Please wait." 

    Any advice or support would be appreciated!

    The form on the website you mentioned belongs to a different JotForm user. If you are in communication with the form owner can you please relay to them your issue in using the form. It will be better if the form owner will be the one to contact us regarding the issue of the form on their account.
