I'm checking your email address but I can't find a JotForm account related to it. May we know what is the username of your JotForm account?
On the integration, you need to provide your Salesforce login credentials and security token. You could also try enabling the API access to see if that will make a different. But please make sure that the type of fields from JotForm matches the one in your Salesforce account leads. This guide might also help: https://www.jotform.com/help/321-Mastering-SalesForce-with-JotForm.
We'll wait for your response.

edition of Salesforce, so I don't think API is enabled by default and will
cost me to enable it. What other solutions are there to integrate JobForm
with SF Professional edition?

Our integrations are based on Web Services API, I checked Salesforce and I found this link: https://help.salesforce.com/articleView?id=000005140&type=1. As mentioned there, the "Professional Edition" does not have the API enabled by default.
Unfortunately, other 3rd party apps other than JotForm won't be able to connect to your account as the limitation is on the account itself. I suggest contacting Salesforce support team first to confirm this.
If you want to try other ways on integrating JotForm to Salesforce, you can try using Zapier, here's a link to it: https://zapier.com/zapbook/jotform/salesforce/. I am not exactly sure how Zapier sets this up, but the integration happens on their end.
I hope that helps.
