new form is disabled

    Hi My first form is appearing as disabled although the form is labelled as enabled?

    I checked your account and I noticed it is auto-suspended by our anti-phishing filter, I have also checked your form and I found it is built in accordance with our  Terms of Use . In sight of that, I reactivated the account as suspension was due to an error.
