We have several reservations forms on our site and none of them are appearing on their respective pages.

I have checked your account and it is in good status. I check your forms using the form direct link and it is working fine. Then, I checked the website URL you provided (https://barleymow.com/locations/westboro/westboro-reservations/) and it is working properly.
