Cancel my Jotform form

I am the president of Beaver Crating & Specialty Services.  We currently have your form on our website and need to cancel our order and stop the monthly payments.

I do not remember my log in or password.  

How do I cancel this order?

Could you please give us anything related to that account, form ID, or where the form is embedded in your website since the link you provided is not working and I wasn't able to locate any paid account under your email address.
