Form fields are not aligned correctly.

    this does help! I'm noticing that my questions "HL Number" "Title" "Quantity" "Price" are not in aligned 4 columns after the first line. Can you direct me to the setting to correct that? I appreciate it!

    I assume that "HL Number" fields should be first fields of each row. To align your fields properly you will have to set Move to a new line option to ON for all of your "HL Number" fields.
