    How can I have the form to include the text field in the notification email?

    How do I get an entire form sent? 

    We are trying to create a doc with signature at the bottom. When signed by receiver, we want to get a copy of the entire document. Its now sending me only signature.

    Text fields are not automatically added to the email template. You need to do it by adding a new row manually. You can use the table tools in the email template:


    Or, enable the PDF attachment in the form to get a PDF copy of the submission. 

    In order for the text field to show up in the PDF, you need to check Show Headers and Texts in the form submissions page settings:
