Is there an iphone bug with completing forms?

We have a form to receive submissions for a scholarship and for users on the iPhone, the form is impossible to complete. When you start typing in an entry field, the form scrolls down to the bottom and you're no longer able to see the field. Then when you hit submit, the form jumps to the top and you aren't able to complete the submission process. We made a video of the experience.

Can you please try re-embedding your form using the iFrame embed code and see if this helps to solve the issue you are encountering?
