All of my forms sayForm not foundThis form is disabled.

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I see that the account is suspended due to the form https://www.jotform.us/form/70813842084153 which is intended to collect credit card information on the form.
Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.
In order to activate your account, it is required for you to accept our terms of usage and agree to delete the guilty fields from the form. Please confirm so that we can activate the account.

The fields have been deleted.

I notice that there are text fields asking the users to fill the credit card information on the form though the fields are deleted. Please remove the text of credit card as well since our anti-phishing filter will be detecting as a phishing form and the account will be suspended again.
I have now activated your account and you should be now able to login to your account to edit the form and remove the text to avoid any further suspensions on your account.
