Form with Operations

Buen dia 

tengo una duda, si necesito crear un fromulario donde se integren operaciones  como resta suma etc. Esto es posible??  como lo hago?

me pueden apoyar , qeudo en espera de sus comentarios

Sorry for that I could not understand your problem clearly. Do you want to calculate a total based on the option selected by the user from your form?
