Form Error. Form has been Disabled

Hi,

I have made 2 forms. both of which are to collect information from venue promoters for gigs and information from the band i work with.

Why have they been disabled? - they are there for essentially research and communication purposes and no other reason.

please can you re-instate the account to access these forms.
