How can I make a single choice AND allow them to fill in the blank if they select that one?

    Hello. If you would like to have in a list of options to be able to only select one item, you have to make use of the radio button field. With a list of items that you can provide the user can only be allowed to select one.

    Another field you can use is the Dropdown field. By default, users are only allowed to select one item in the list that you can provide in that field

    What is not clear to me however is what you mentioned that allowing them to fill in the blank. On radio button (single choice) fields, you can add an Other option which will then compel users to enter a value next to it

    But if what you mean is that you would like to show up a field after selecting one item and that a user should fill up the field that showed up, you can make use of the show/hide conditions of your form.     But if none of these is what you mean, it would be great if you can provide us more detail on what you wish to achieve, so we can provide you with better details.

    I am trying to do something similar, a single choice WITH a fill in the blank: see example

    A) Benefits begin the___ day of the month following___  days of eligible employment.

    B) Benefits begin the first day of the month following___ months of eligible employment.
