Hello. If you would like to have in a list of options to be able to only select one item, you have to make use of the radio button field. With a list of items that you can provide the user can only be allowed to select one.
Another field you can use is the Dropdown field. By default, users are only allowed to select one item in the list that you can provide in that field
What is not clear to me however is what you mentioned that allowing them to fill in the blank. On radio button (single choice) fields, you can add an Other option which will then compel users to enter a value next to it
But if what you mean is that you would like to show up a field after selecting one item and that a user should fill up the field that showed up, you can make use of the show/hide conditions of your form.
