How to create form with similar design?

    i also have another questions pls how can come out with this design ie https://www.applyboard.com/schools with jotform, even similar. Thank

    You can make use of our Image Choices widget, or Image Radio Buttons widget, it will allow you to add images to your form with radio button selection, but they would look in a similar way to the example, but please note that with this, you can only pick one choice from the list. 

    Alternatively, you can add links to your form using our TEXT element from the form builder, and configure it with CSS to look similar to your example.

    Hello Emmy,

    I did check the sample webpage which you have shared and found that the boxes is not an image but it has image with content. I believe it is possible to achieve your requirement by adding "Text" fields in your form for each boxes.

    I am creating a demo form form and will get back to you as soon as I am able to complete it.

    its pls. thank you, so i should clone the url of the one u have created , and then can i then edit the picture and insert mine own picturesPls guide me if am missing the directionsor should i copy the real source of the site ie, applyboard.com or Demo Form, how can i start to create my form.

    Hello  Emmy,

    You can clone the form in your account and then start editing the content. Please note that since the HTML code is added in text field to match the look & feel of previous one, you will need to make all the changes in the text field only.
