Field labels are not shown in the form

    Many of our forms are no longer showing the field identifying text portion of the form, just the text fields. So our clients only see the boxes, so they don't know which fields they are filling out.

    I have included a link of a sample, but it's not just one form. It's most of our forms, and it all started 2 days ago. 

    Please see screen shot attached. 

    We love Jotform, but our clients are getting upset.

    We would like to apologize for any inconvenience this is causing you. The field labels on your form(s) are set to be hidden base on the theme that was applied to the form. To rectify the issue, I would suggest you add the following CSS codes to your forms.

    .form-label {

        display: inline-block !important;

    }
