How do I fix this dropdown checkbox quantity selector on my order form?

I don't know how this went all wrong.  I was trying to get the "Place your order here!" text centered, and fooling with trying to center either my products or the underlined labels (really "free" products), and now on certain mobile devices and browsers I had this weird display where some of the products have the quantity dropdown even when the checkbox is unchecked, when it should be collapsed.  And on my iPhone it also has this ugly cutoff.

Here's a screenshot from my phone.  I can't figure out in the relatively small amount of accurate CSS I injected how this happened.

Also, how can I center "Place your order here!"? :)

Thanks so much!  Here's my form

Have you managed to fix the issue? The form seems to be fine on the mobile devices at the moment.

Nope, didn't fix it :(.  Here's a screenshot from Safari on my son's Mac.  It works on my Mac in Chrome, but in Safari I have the problem on a different mix of flavors.  Guess I'll keep messing around and see if I can find the problem.

I have checked the form using Android phone, iPhones (using official simulator app), macOS Sierra (Safari 10), macOS El Capitan (Safari 9), but still I was not able to reproduce the issue.

Just in case I have cleared your form caches.

If you use any special settings or perform any specific steps to reproduce the issues, please let us know.

Ok, thanks anyway :).  I'll try not to stress if it's not likely customers will see it.
