We have had feedback from our customers who are unable to fill out our JotForm time slot page on both desktop and mobile devices.
I too have tried completing the form on desktop and mobile device and text does not appear in the text box, and checkboxes do not work either.
Can you please let me know how this can be resolved?

The issue is caused by the current theme you are using.
Please remove the theme to fix the issue.
1. Click on the Form Designer icon
2. Themes > Remove Theme
