Why my form has been disabled?

    When i click the link it shows as disabled, however this is not the case and it is enabled. Please help. The form link is

    I have checked your account and I can see it has been disabled since you're asking for info that goes against our terms on the following forms:

    Please, do note that asking for accounts or passwordis not allowed on JotForm, I would suggest you to also read our terms in order to know what you cannot ask on your forms and avoid to get your account suspended again: http://www.jotform.com/terms/ 

    In order to reactivate your account I removed those guilty fields from your forms, please check and let us know if you have any question.

    These fields are for their Groupon account not for jot form. We need to know what they want their preferred login and preferred password to be in order to set up their account. Is there a better way we should say this on the form?

    We are creating their merchant account so need to know what they want for a log in email and preferred password. Make sense?

    We understand that you need the fields, but unfortunately, we cannot make exceptions and allow to ask for password or usernames in JotForm forms. 

    If you will use the email address as username then you could ask only for this info, then once you have the email address you could contact your user directly in order to ask for the password, otherwise, as explained above, our anti-phishing system will detect the activity and even if you're not asking login info for existing accounts, you will be suspended again.
