Why is my form giving an error message saying it is disabled?

I created a new form today, Mainstream Life Hacks, and when I try to preview it or go to the live form it says "Form not found. This form is disabled" 

I have checked the settings and the form is enabled. I have tried checking and unchecking the setting. I have tried disabling and re-enabling through the "more" options in the "My Forms" list.

Why does the form show as disabled? How can I enable it and use it?
