How can I insert text into a field based on previous fields selections?

    I am hoping you can tell me how to perform what I believe is a somewhat unusual task.

     

    I want to create a form where users select one sentence from a set of four sentences. They do this for five different sets of sentences, so that they have a total of five sentences. The piece I am missing is having those five sentences appear in a text box that the user can then edit before submitting. Is there a way to do this? Can you help me do this, or point me at resources that can help me figure itout on my own?
