Can you add a JotForm to a SmugMug web site?

If I were to use a 'Starter' level form, would I be able to create a form with many fields and 2 image files upload capability?

You will be able to embed a JotForm as long as they allow to embed custom iFrame or JavaScript code, you could take a look on this guide in order to know which embed code you could use: Which-Form-Embed-Code-Should-I-Use

Yes, there is not a limit on the number of fields you could add to your form.
