My forms are disabled?

Mine are disable too for some unknown reason. it was working the whole time and I wad checking the previews and now all of the forms say disabled when I try to view them.

When I checked your account, I found out that your account has been auto-suspended by our anti-phishing system due to password field on your forms. 

Please note that asking personal information such as password as in your form, is strictly forbidden due to violation of our terms of use. 

So, I deleted password field from two forms and activated your account. Your forms are active now. If you insist on collecting such data, your account will be suspended again.
