My embedded form cannot be displayed: Form not found

I've contact you about a form that desapear...

Hi,

When I check your form which is titled as "Contact rapide : California Limousines", I have found it in trash folder. 

Please sign in your Jotform account and go to the "Trash" folder. Then please restore your form. 

Note: Please be careful not to purge it. If you purge it, this operation cannot be undone.
